Bibliographic research was systematically conducted through theoretical readings surrounding the Unitary Theory of Social Reproduction from the perspective of historical materialism, which theoretically and methodologically supported the object. Empirical research was developed through the analysis of the researcher's field diaries, and documentary research examined social interviews, norms, institutional productions, as well as current legislation on the subject. The results revealed that the reality of HUOL translates, at a micro-level, the various ways in which neoliberalism, the current stage of capitalism, has intensified its impact on the work carried out by women. This is reflected in the ideological understanding of care work as a cost rather than an investment. The data shows that HUOL appropriates a portion of the unpaid work carried out by women companions, work that is performed through caring for the users, while marginalizing the health demands of these companions in the pediatric sector. O objeto de estudo deste trabalho são as atividades de cuidado desenvolvidas por mulheres acompanhantes de usuárias(os) hospitalizadas(os) nas Enfermarias Pediátricas e na UTI Pediátrica do Hospital Universitário Onofre Lopes - HUOL/ UFRN. A pesquisa bibliográfica foi realizada de maneira sistemática através de leituras teóricas em torno da Teoria Unitária da Reprodução Social sob a perspectiva do materialismo histórico, que sustentou em termos teórico-metodológicos o objeto. A pesquisa empírica foi desenvolvida por meio de análise dos diários de campo da pesquisadora e a pesquisa documental examinou entrevistas sociais, normativos e produções institucionais, bem como a legislação vigente em torno do tema. Os resultados evidenciaram que a realidade do HUOL traduz em nível micrológico as diferentes maneiras como o neoliberalismo, atual estágio do capitalismo, vem recrudescendo seu impacto sobre o trabalho desenvolvido pelas mulheres, o que se traduz em apreender ideologicamente os trabalhos de cuidado como custo e não investimento.
