Resumo:Objectives: To evaluate gestational weight gain using both the Institute of Medicine (IOM) and Kac and Carrilho recommendations, seeking to identify the difference in the classification of total weight gain in pregnant women. Methodology: Conducted as a cross-sectional study, the research involved pregnant women over 18 years of age, who underwent their first prenatal consultation in Primary Health Care centers in Sousa-PB. Various maternal data, such as weight, height, pre-pregnancy weight and gestational age, were collected. Pregnancy weight gain up to the moment of the consultation was assessed based on both the IOM recommendations and those of Kac and Carrilho suggested by the Ministry of Health in the pregnant woman's health record. Agreement between classifications was analyzed using the Kappa statistic, with a significance level of 5%. Results: Of the 48 pregnant women evaluated, 64.6% were overweight in their pre-pregnancy nutritional status. According to IOM recommendations, 22.9% had weight gain above the recommended level, while according to Kac and Carrilho, this percentage increased significantly to 68.8%. The Kappa statistic of 0.073 and the p-value of 0.280 indicate the lack of agreement between the classifications of the two references, highlighting the sensitivity of these guidelines in identifying pregnant women with excessive gain. Conclusion: The application of the recommendations proposed by Kac and Carrilho resulted in a substantial reclassification of pregnant women in relation to gestational weight gain, highlighting the sensitivity of these guidelines in identifying pregnant women with excessive weight gain. The lack of agreement between classifications highlights the importance of healthcare professionals being up to date with the most recent guidelines and incorporating them into their clinical practices.
