Resumo:Introduction: The International Classification of Functioning, Disability and Health considers functionality to be influenced by the complex interaction between existing diseases and environmental and social factors based on the biopsychosocial model of care. Thus, the Model Disability Survey (MDS) is a tool created by the World Health Organization (WHO) in order to assess the functionality and disability profile of a population. It was translated, adapted and validated in Brazil in 2021, also indicated for epidemiological use, standing out for having a more detailed analysis proposal. Objective: To describe the prevalence of health conditions and their association with sociodemographic characteristics and functionality in the adult and elderly population of Santa Cruz, Rio Grande do Norte. Methods: This is an observational and cross-sectional study with data collected through a population survey. The survey was carried out in the municipality of Santa Cruz between October 12 and November 1, 2022 in different households with an average duration of 48 minutes, using tablets. The database was automatically updated in the search system. People aged 18 years or over, of both sexes, residing in the municipality were included, and participants unable to understand and/or respond were excluded. Results: Among the 504 people who participated in the study, 98.6% declared themselves to be Brazilian, 76.4% female, 44.6% self-declared brown, 38.3% married and 17.6% were between 4-8 years of study. Among the three aspects of the Functionality module, the highest prevalence for severe disability was Activity and Participation, comprising 28.2% of the population. Pain was reported by 38.3% of people with severe disability. The most prevalent health condition, diagnosed, using medication and other treatments, was Systemic Arterial Hypertension with 35.3% prevalence. If associated with sociodemographic conditions, women diagnosed with arthritis/arthrosis were the most prevalent finding, representing 98.4% of people diagnosed. The condition was more prevalent among married people (37.5%) and self-declared white (42.2%). Discussion: The health condition Systemic Arterial Hypertension was the most frequent finding in women, and among the self-declared white, brown and black population, severely compromising functionality in participation in society, cognition and mobility. In addition, among the groups evaluated regarding presence, diagnosis, medication and treatment, SAH is highlighted, associating frequent condition of inappropriate use of medication and commitment to perform activities of daily living, which can lead to a decrease in functionality. In the Activity and Participation domain, those affected by severe disability have problems with using their hands, self-care, interpersonal relationships, performing household chores, participating in the community, caring for others, and working and studying. Conclusions: The construction of these data, which associated health conditions with functionality, can subsidize the improvement of public policies from the recognition of the most common problems buoyed by a biopsychosocial collection instrument accepted worldwide, thus being able to serve as a parameter for national and international studies.
