Cystic fibrosis (OMIM #219700) is a severe, multiorgan genetic disease
that affects mucus and sweat-producing cells, primarily causing chronic lung
infections, pancreatic insufficiency, and high levels of sweat chloride. It
originates from variants in the gene that encodes the cystic fibrosis
conductance regulatory protein (CFTR), being more frequent in white people.
The identification of the CF pathogenic variants allows the establishment of
precision medicine based on new therapies that modulate the CFTR protein.
The present study aimed to describe the clinical and molecular profiles of 37
cystic fibrosis patients from a referral center in Rio Grande do Norte. Saliva
samples were collected from 37 patients with cystic fibrosis, DNA was
extracted, about 50 ng of DNA was enriched with the Nextera Custom Rapid
Capture Kit (Illumina, San Diego, CA, USA), and the high throughput
sequencing (HTS) was performed through a HiSeq 4000 (Illumina). Eleven
different pathogenic variants were found, with c.1521_1523delCTT being the
predominant one in 27 cystic fibrosis patients (73%) (54.1% homozygous and
18.9% compound heterozygous). Among the patients with at least one allele
with c.1521_1523delCTT, 25 can benefit from CFTR modulators therapies
based on correctors and potentiators of the CFTR protein: 5 patients aged 2 to
5 years can be treated with the double combination lumacaftor and ivacaftor
and 20 patients aged 6 years old and over can be treated with the triple
combination elexacaftor, tezacaftor and ivacaftor. Therefore, personalized
treatment will reflect on better control of comorbidities associated with cystic
fibrosis, as well as on the prognosis of patients who may have quality of life and
increased life expectancy. |

A fibrose cística (OMIM #219700) é uma doença genética grave, multiorgânica, que afeta células produtoras de muco e suor provocando principalmente infecções pulmonares crônicas, insuficiência pancreática e altos níveis de cloreto no suor. É causada por variantes patogênicas no gene que codifica a proteína reguladora da condutância da fibrose cística (CFTR), sendo mais frequente em pessoas de pele branca. A identificação das variantes causadoras da fibrose cística permite a instituição de uma medicina de precisão a partir das novas terapias moduladoras da proteína CFTR. O presente estudo teve por objetivo descrever os perfis clínico e molecular de pacientes com fibrose cística de um centro de referência do Rio Grande do Norte. A coleta de saliva com swab foi realizada em 37 pacientes com fibrose cística, o DNA foi extraído, e cerca de 50 ng de DNA foram enriquecidos com o Kit Nextera Custom Rapid Capture (Illumina) e o sequenciamento de alto rendimento (HTS - do inglês, High Throughput Sequencing) foi realizado em uma plataforma HiSeq 4000 (Illumina). Foram obtido os genótipos em 31 de 37 (83,8%) pacientes com FC. Foram encontradas onze variantes patogênicas diferentes, sendo a c.1521_1523delCTT a predominante em 27 pacientes (73%) com fibrose cística (54,1% em homozigose e 18,9% em heterozigose composta). Entre os pacientes com pelo menos um alelo com c.1521_1523delCTT, 25 podem ser beneficiados com as terapias moduladoras à base de corretores e potencializadores da proteína CFTR: 5 pacientes de 2 a 5 anos podem ser tratados com a combinação dupla lumacaftor e ivacaftor e 20 pacientes acima de 6 anos podem ser tratados com a combinação tripla elexacaftor, tezacaftor e ivacaftor.
