description Introduction: Maxillary expanders are positioned centrally on the palate. Thus, these appliances might interfere on tongue articulation and impact negatively the speech of patients undergoing rapid maxillary expansion (RME). Objectives: To evaluate changes on speech sound production in children, adolescents and adults treated with RME. Material and methods: Two reviewers independently and blindly performed an electronic search. The articles were pre-selected based on the title and abstract. After reading the fulltext articles, the studies that met the eligibility criteria were included. A manual search was performed by examining the references of each selected article. The ROBINS-I tool was used to qualitatively assess the selected studies. Results: Ten articles were selected, and the risk of bias ranged from moderate to critical. The studies showed methodological limitations related to the selection of eligibility criteria, lack of randomization and allocation concealment of participants, lack of control groups, lack of assessment blinding and lack of standardization of the type of expander used, the amount of expansion and the follow-up time. The studies found a tendency of RME promotes significant changes in speech. However, the negative impact on speech sounds production seems to be temporary, tending to improve during the retention phase and returning to normal after the maxillary expander removal. Conclusion: Rapid maxillary expansion seems to promote temporary changes in speech sound production that return to normality after expander removal. However, the quality of the evidence is limited, and well-conducted randomized clinical trials are necessary to better elucidate these effects.

Introdução: Os expansores maxilares ficam posicionados centralizadamente no palato. Assim, é possível que o aparelho interfira na articulação da língua e influencie negativamente na produção dos sons da fala de pacientes submetidos à expansão rápida da maxila (ERM). Objetivos: Avaliar as alterações na produção dos sons da fala em crianças, adolescentes e adultos submetidos a ERM. Material e métodos: Duas revisoras realizaram uma busca eletrônica de maneira independente e cega. Os artigos foram pré-selecionados com base na leitura do título e do resumo. Após a leitura dos artigos na íntegra, os estudos que preencheram os critérios de elegibilidade foram incluídos. Uma busca manual foi realizada examinando-se as referências de cada artigo selecionado. A ferramenta ROBINS- I foi utilizada para avaliar qualitativamente os estudos selecionados. Resultados: Dez artigos foram selecionados e o risco de viés variou de moderado a grave. Os estudos apresentaram limitações metodológicas relacionadas à definição dos critérios de elegibilidade, falta de randomização e de ocultamento de alocação dos participantes, ausência de grupos controles, falta de cegamento dos avaliadores e falta de padronização do tipo de expansor utilizado, da quantidade de expansão e do tempo de acompanhamento. Os estudos encontraram uma tendência de a ERM promover alterações significativas na fala dos pacientes. No entanto, a influência negativa na produção dos sons da fala parece ser temporária, tendendo a melhorar durante a fase de contenção pós-expansão e retornando aos parâmetros acústicos normais após a remoção do expansor maxilar. Conclusão: A ERM parece promover alterações temporárias na produção dos sons da fala, a qual retorna à normalidade após a remoção do expansor.
