| Resumo: | The basis of any health care is recorded in medical records, in which health information in the care process serves the health professional to determine the appropriate conduct. This work aims to propose a guide protocol to plan, organize and implement an electronic medical record considering the flows of care and organization of data from clinical schools at UFRN. The construction of the protocol was given by the Double Diamond methodology, following the four steps: Discovery, Definition, Development and Delivery. The protocol resulted in a categorization of three types of data: Socioeconomic, Basic Health and Care. This division optimizes the care processes and collaborates in a more organized structure within the medical record, facilitating the correct identification of the type of data that the Health Professional needs. The protocol was implemented in the SigSaúde project and is being used by some UFRN teaching clinics. |
