description One of the tools used by nurses in their work process is the Nursing History, the first stage of the Nursing Process, known as the research phase, which consists of collecting data to identify real or potential problems of people who need care. For data collection, the nurse needs an instrument that will serve as a guide and that reproduces the needs of the person being cared for. The objective of this study was to build and validate a Nursing History instrument for a Pediatric Intensive Care Unit of a University Hospital. It is a methodological study, with a quantitative approach that used Wanda Horta's Theory of Basic Human Needs as a theoretical framework and Pasquali (2010) as a methodological framework, which emphasizes the importance of the instrument validation process. The study it was be developed from three poles or stages: the first consisted of the Theoretical polo, with a scoping review in order to select the results to compose the construction items of the instrument for Nursing History, guided by the framework of the Theory of Basic Human Needs by Wanda Horta and construct elaboration; the second, consists of empirical procedures, was based on the judgment of the content and appearance of the construct by 15 judges, experts in the field of Nursing, using a Likert-type scale and semantic evaluation of items and domains through the Brainstorming technique; the third consists of statistical procedures with statistical analysis and validation of the final version of the instrument through the content validity index (CVI) and Kappa concordance coefficient (k). As a result, six studies were obtained in the first pole, which made up the final sample of the scoping review and through which the first version of the instrument was organized, with seven domains and 78 items. In the second pole, the Judges and experts validated the content and appearance using a Delphi technique round, where two items obtained an agreement index below the cutoff point (<0.64). The instrument did not change after the semantic assessment of the items and domains using the Brainstorming technique, since the participants demonstrated understanding of all items, with agreement above the cutoff point (>0.70) for all items. At the end of the evaluation, the final instrument contains seven domains and 79 validated and understandable items to be used in the Pediatric ICU. It is believed that the Nursing History instrument contributes to improving the quality of Nursing care provided to patients admitted to a Pediatric ICU, in addition to optimizing the time spent by the professional in nursing care and favoring communication between team members.

Uma das ferramentas utilizadas pelo enfermeiro no seu processo de trabalho é o Histórico de Enfermagem, primeira etapa do Processo de Enfermagem, conhecida como a fase de investigação, que consiste na coleta de dados para identificação de problemas reais ou potenciais das pessoas que necessitam de cuidados. Para a coleta de dados o enfermeiro necessita de um instrumento que lhe sirva de guia e que reproduza as necessidades da pessoa a ser cuidada. Objetivou-se nesse estudo construir e validar um instrumento de Histórico de Enfermagem para Unidade de Terapia Intensiva Pediátrica de um Hospital Universitário. Tratase de um estudo metodológico, com abordagem quantitativa que utilizou como referencial teórico a Teoria das Necessidades Humana Básicas de Wanda Horta e como referencial metodológico Pasquali (2010) que enfatiza a importância do processo de validação do instrumento. O estudo foi desenvolvido a partir de três polos ou etapas: o primeiro constituiuse do polo Teórico, com uma revisão de escopo a fim de selecionar os resultados para compor os itens de construção do instrumento para Histórico de Enfermagem, guiado pelo referencial da Teoria das Necessidades Humanas Básicas de Wanda Horta e elaboração do construto; o segundo, formado pelos procedimentos empíricos, fundamentou-se no julgamento do conteúdo e aparência do construto por 15 juízes experts na área da Enfermagem, utilizando uma escala tipo Likert e avaliação semântica do itens e domínios através da técnica do Brainstorming; o terceiro consiste dos procedimentos estatísticos, com a análise estatística e validação da versão final do instrumento por meio do índice de validade de conteúdo (IVC) e coeficiente de concordância de Kappa (k). Como resultado, obteve-se no primeiro polo seis estudos que compunham a amostra final da revisão de escopo e pelos quais organizou-se a primeira versão do instrumento com sete domínios e 78 itens. No segundo polo, os Juízes e experts validaram o conteúdo e a aparência utilizando uma rodada da técnica Delphi, onde dois itens obtiveram um índice de concordância abaixo do ponto de corte (< 0,64). O instrumento não sofreu alterações após a avaliação semântica dos itens e domínios pela técnica Brainstorming uma vez que os participantes demonstraram compreensão de todos os itens, com concordância acima do ponto de corte (> 0,70) em todos os itens. Ao término da avaliação, o instrumento final contém sete domínios e 79 itens validados e compreensíveis para ser utilizado em UTI Pediátrica.
