It is a problem that affects 30% to 50% of the general population and is therefore an important
reason for spending on health. Manual therapy together with exercise therapy are widely used
approaches in the treatment of CCNE, requiring studies on their effects and methods of use.
Objective: to verify the evidence on manual therapy combined or not with exercise therapy in
patients with CCNE. Methods: This work is characterized as a narrative review of
randomized clinical trials published until September 2022. The searches were carried out in
the PEDRO, PUBMED and LILACS databases. Articles that used some manual therapy
technique, associated or not with physical exercise, were included. Results: After excluding
duplicates, analyzing titles and abstracts, 7 articles published between 2014 and 2022 were
included in this review, totaling 485 participants (364 women and 121 men). Maitland was the
most used technique, the most common outcomes were pain intensity and cervical disability,
measured mainly by visual scale and cervical disability index, respectively. In general,
manual therapy associated with an exercise program proved to be more effective in reducing
neck pain and disability if the use of these two resources was discovered separately. The same
occurred for the variables: disease severity and global improvement, cervical range of motion,
perceived global recovery and quality of life. Conclusion: Manual therapy and exercise
therapy were found to be effective in the treatment of CCNE when applied clinically, but the
results were better when used together. |

Introdução: A cervicalgia crônica não específica (CCNE) é caracterizada pela dor na região do pescoço sem etiologia definida e com duração superior a 12 semanas do aparecimento dos sintomas. É um problema que acomete de 30% a 50% da população em geral e por isso é um importante motivo de gastos com a saúde. A terapia manual juntamente com a terapia de exercícios são abordagens muito utilizadas no tratamento da CCNE, necessitando que se estude sobre seus efeitos e métodos de utilização. Objetivo: verificar as evidências sobre a terapia manual combinada ou não a terapia de exercícios em pacientes com CCNE. Métodos: Este trabalho caracteriza-se como uma revisão narrativa de ensaios clínicos randomizados publicados até setembro de 2022. As buscas foram realizadas nas bases PEDRO, PUBMED e LILACS. Foram incluídos artigos que utilizassem alguma técnica de terapia manual, associada ou não com o exercício físico. Resultados: Após a exclusão das duplicatas, análise de títulos e resumos, 7 artigos publicados entre 2014 e 2022 foram incluídos nesta revisão, totalizando 485 participantes (364 mulheres e 121 homens). Maitland foi a técnica mais utilizada, os desfechos mais recorrentes foram a intensidade da dor e incapacidade cervical, medidos principalmente pela escala visual analógica e índice de incapacidade cervical, respectivamente. De modo geral, a terapia manual associada a um programa de exercícios mostrou-se mais eficaz na diminuição da dor e da incapacidade cervical se comparada a utilização desses dois recursos em separado. O mesmo ocorreu para as variáveis: gravidade da doença e melhora global, amplitude de movimento cervical, recuperação global percebida e qualidade de vida.
