Resumo:Objective: To determine the main adverse reactions associated with the use of tacrolimus after kidney transplantation and to relate the occurrence of these reactions to factors intrinsic to the drug and post-transplantation therapy. Methods: This is a systematic review carried out in PubMed electronic databases, using the POT approach (population, outcomes and type of study) as a search strategy. Exclusion criteria were reviews, meta-analyses, in vitro / in vivo studies, opinion articles, non-free original articles and studies on transplantation of other organs or combined. Results: Diabetes mellitus (5.73%), dyslipidemia (3.64%), osteopenia/osteoporosis (2.63%), cardiovascular events (2.11%) and dyspepsia (1.66%) were the most frequent adverse reactions prevalent in the 3731 patients evaluated in the ten observational studies included. There was predominance of the triple immunosuppression scheme (tacrolimus, mycophenolic acid and corticosteroids). The daily dose of tacrolimus was 5.1 ± 2.4 mg/day and the average use time was 24 months, with serum concentration decreasing from 9.14 ng/mL to 5.8 ng/mL depending on the time of transplantation. Conclusion: Tacrolimus significantly altered outcomes after kidney transplantation, reducing rejection rates and increasing graft survival. However, high doses of the drug are associated with a series of adverse reactions.
