Primary Health Care is one of the levels of the Brazilian Unified Health System,
characterized by health promotion, prevention and protection actions. Primary health
care has a direct link with public health policies as it is regulated by the National
Primary Care Policy. Among the subjects that can be addressed in public policies,
the sleep/wake cycle can be highlighted. The damage to health caused by changes
in relation to this cycle are increasingly evident. Therefore, the objective of this study
was to map evidence on the sleep/wake cycle related to health policies in the context
of primary health care. This is a scoping review conducted according to the Joanna
Briggs Institute methodology and PRISMA-ScR. The databases consulted were
PubMed, LILACS and Scopus, in English and Portuguese publications from the last 5
years. The primary care notebooks on the website of the Secretariat of Primary
Health Care of the Brazilian Ministry of Health were also consulted. It was found 990
evidence, 959 articles came from the databases, of which 28 were selected for full
reading, and 1 article was found by recommendation, thus 9 articles were included in
the study. Among the evidence, 30 were primary care notebooks and of these only
23 were included. The 9 articles analyzed were mostly from North American and
European countries and addressed health policies related to: insomnia; sleep apnea;
quality of life improvement course; sleep education program; sleep clinic; compliance
with national guidelines; life habits and academic performance; and the delay in
starting classes. In primary care notebooks, it was observed that 9 guide health
professionals to investigate or question patients about sleep during consultations and
some inform what professionals should advise about sleep. In addition, sleep or
drowsiness was observed in 8 notebooks as an adverse effect of the use of drugs
and 10 of them indicated changes in sleep or drowsiness as symptoms resulting from
health conditions. It is clear then that the central theme addressed in the articles and
notebooks evaluated in this study refer specifically to sleep, treating in a limited way
evidence about the sleep/wake cycle and biological rhythmicity in health policies of
primary health care. In addition, no health policy related to the sleep/wake cycle was
observed in Brazil. In a positive way, it is observed that sleep is a topic addressed in
primary care notebooks, but it does not integrate or address the biological rhythm of
the most varied body systems, which is fundamental for individual and collective
health, as well as the relationship between sleep/wake cycle with health-diseaseillness processes. It is hoped that this review can provide evidence for the
development of new studies and health policies on the sleep/wake cycle in Brazil and
in the world and serve as a basis for the implementation of health promotion and
disease prevention actions and strategies. |

A Atenção Primária à Saúde é um dos níveis do Sistema Único de Saúde brasileiro, caracterizada por ações de promoção, prevenção e proteção da saúde. A atenção primária à saúde possui ligação direta com as políticas públicas de saúde por ser regulamentada pela Política Nacional de Atenção Básica. Dentre as temáticas que podem ser trabalhadas em políticas públicas evidencia-se o ciclo sono/vigília. Os danos à saúde provocados por alterações em relação a este ciclo são cada vez mais evidentes. Portanto, o objetivo deste estudo foi mapear evidências sobre o ciclo sono/vigília relacionadas às políticas de saúde no contexto da atenção primária à saúde. Trata-se de uma revisão de escopo conduzida de acordo com a metodologia do Joanna Briggs Institute e com o PRISMA-ScR. As bases de dados consultadas foram PubMed, LILACS e Scopus, sobre publicações dos últimos 5 anos, em inglês e português. Também foram consultados os cadernos de atenção básica que constam no site da Secretaria de Atenção Primária à Saúde do Ministério da Saúde brasileiro. De 990 evidências encontradas, 959 artigos foram provenientes das bases de dados, dos quais 28 foram selecionados para leitura completa, e 1 artigo foi incluído por recomendação, assim 9 artigos foram incluídos no estudo. Dentre as evidências, 30 eram cadernos de atenção básica e destes apenas 23 foram incluídos no estudo. Os 9 artigos analisados eram na maioria decorrentes de países da América do Norte e Europa, e abordavam políticas de saúde relacionadas à: insônia; apneia do sono; curso de melhoria da qualidade de vida; programa de educação sobre o sono; clínica de sono; cumprimento de diretrizes nacionais; hábitos de vida e desempenho acadêmico; e o atraso no início das aulas. Nos cadernos de atenção básica foi observado que 9 orientam os profissionais de saúde a investigarem ou questionarem os pacientes sobre o sono durante os atendimentos e alguns informam o que os profissionais devem orientar sobre o sono. Somado a isso, foi observado em 8 cadernos o sono ou a sonolência como efeito adverso ao uso de fármacos e 10 deles apontavam alterações no sono ou sonolência como sintomas decorrentes de condições de saúde. Percebe-se então que o tema central tratado nos artigos e cadernos avaliados neste estudo refere-se ao sono especificamente, tratando de forma limitada evidências sobre o ciclo sono/vigília e a ritmicidade biológica nas políticas de saúde da atenção primária à saúde. Além disso, não foi observada política de saúde relacionada ao ciclo sono/vigília no Brasil. De forma positiva, observa-se que o sono é tema abordado nos cadernos de atenção básica, porém não integra ou aborda a ritmicidade biológica dos mais variados sistemas corporais, a qual é fundamental para a saúde individual e coletiva, assim como, a relação do ciclo sono/vigília com os processos saúde-doençaadoecimento.
