Objective: to carry out a literature review on the clinical intervention actions of the pharmacist in the care of kidney transplant patients. Methodology: integrative review carried out from January to March 2022 with studies published in the last 5 years in the main databases PubMed/MEDLINE, ScienceDirect, Scopus, EMBASE and Web of Science in the English language. The acronym PICO was used, which consists of: Population; Intervention; Comparison and Outcome. Results: the search in the databases resulted in a total of 4,358 published articles and only 10 were selected to compose the review, according to the eligibility criteria. Among the included studies, we observed predominantly male and non-elderly patients. Conclusion: It was possible to observe the actions of clinical intervention of the pharmacist in the care of kidney transplant patients in the 10 selected studies as well as the effective integration of this professional in the interprofessional teams of kidney transplantation. Different types of pharmaceutical interventions were carried out: educational campaigns capable of reducing the use of opioids; increase in patients' knowledge about medication use; there was reestablishment of therapeutic levels in 70% of patients due to improved adherence to pharmacotherapy; reduction of discrepancies through medication reconciliation; reduction in the use of prophylactic antibiotic therapy; there was a 50% reduction in hospitalization rates and also in hospital and medication costs; control of glycemic levels reduced almost 90% of hospitalizations for post-transplant hyperglycemia.

Introdução: os avanços dos serviços clínicos desempenhados pelo farmacêutico, permitiram a sua participação no cuidado individualizado dos pacientes e vem crescendo significativamente e impactando positivamente na promoção, prevenção e recuperação da saúde. Objetivo: realizar uma revisão de literatura sobre as ações de intervenção clínica do farmacêutico no cuidado de pacientes transplantados renais. Metodologia: revisão integrativa realizada no período de janeiro a março de 2022 com estudos publicados nos últimos 5 anos nas principais bases de dados PubMed/MEDLINE, ScienceDirect, Scopus, EMBASE e Web of Science no idioma inglês. Foi utilizado o acrônimo PICO, a qual consiste em: População; Intervenção; Comparação e Outcome/Desfecho. Resultados: a busca nos bancos de dados resultou em um total de 4.358 artigos publicados, sendo selecionados apenas 10 para compor a revisão, conforme os critérios de elegibilidade.Dentre os estudos incluídos observaram-se predominantemente pacientes do sexo masculino e não idoso. Conclusão: Foi possível observar as ações de intervenção clínica do farmacêutico no cuidado de pacientes transplantados renais nos 10 estudos selecionados como também a integração efetiva desse profissional nas equipes interprofissionais de transplante renal.
