description The implementation of the Patient Safety Culture in health services and, more specifically, within the psychiatric hospital, can become a strong ally in the fight for more humanized and safe care. In this perspective, patient safety is seen as an important device regarding more qualified psychiatric care. It is a qualitative research of the Convergent Assistance type. The general objective of this study was the construction of the patient safety protocols for a psychiatric hospital. For that, we sought to: identify the themes considered to be a priority for the team to develop the protocols; verify the theoretical supports that give basis to the themes worked; the definition of innovations necessary for the applicability of the protocols; and, knowing the perception of research participants about the patient safety culture. The investigation took place in a psychiatric hospital in the city of Natal/RN. The sample consisted of 17 professionals, which was made up of the care team and members of the management. Data collection occurred through face-to-face workshops, through the technique of discussion in a Convergence Group, using the Arco de Maguerez methodology. To analyze the data regarding the perception of the patient safety culture, the Collective Subject Discourse method was used. The results of this study were organized into three topics, namely: the characterization of the convergence group; The construction and presentation of the protocols; Interdisciplinary Assistance Protocol for Evasion; and Interdisciplinary Assistance Protocol for Suicide. The conception of the convergence group about the culture of patient safety: five discourses of the collective subject were identified. The present study resulted in important developments for the implementation of protocols and the beginning of the implementation of the risk management culture, namely: the construction of the Adverse Events Notification Form and the Daily Fall Risk Assessment form; training of professionals regarding the application of protocols; training of the nursing staff regarding the use of the Morse Fall Scale; and training of professionals about suicide and about the management of patients with suicidal ideation. The use of a problematizing methodology and the collective construction process were essential for the team's adhesion in the construction and implementation of the products, and for the visualization and conduction of the developments resulting from this research.

A implementação da Cultura de Segurança do Paciente nos serviços de saúde e, mais especificamente, no âmbito do hospital psiquiátrico, pode se tornar um forte aliado na luta por uma assistência mais humanizada e segura. Nessa perspectiva, a segurança do paciente é apontada como um importante dispositivo no tocante a uma assistência psiquiátrica mais qualificada. Trata-se de um estudo qualitativo realizado por meio da Pesquisa Convergente Assistencial, cujo objetivo geral foi a construção dos protocolos de segurança do paciente para um hospital psiquiátrico. Para tanto, buscou-se: identificar os temas considerados prioritários à equipe para a construção dos protocolos; verificar os suportes teóricos que subsidiam os temas trabalhados; a definir as inovações necessárias à aplicabilidade dos protocolos; e conhecer qual a percepção dos participantes da pesquisa sobre a cultura de segurança do paciente. A investigação ocorreu em um hospital psiquiátrico no município de Natal/RN. Fizeram parte da amostra 17 profissionais, que foi composta pela equipe assistencial e membros da gestão. A coleta de dados ocorreu por meio de oficinas presenciais, através da técnica de discussão em Grupo de Convergência utilizando-se a metodologia do Arco de Maguerez. Para análise dos dados referentes à percepção da cultura de segurança do paciente, foi utilizado o método do Discurso do Sujeito Coletivo. Os resultados desse estudo foram organizados em três tópicos, são eles: a caracterização do grupo de convergência; A construção e apresentação dos três protocolos assistenciais de segurança do paciente; A concepção do grupo de convergência acerca da cultura de segurança do paciente, onde identificou-se cinco discursos do sujeito coletivo. O presente estudo resultou em desdobramentos importantes para a implementação dos protocolos e início da implantação da cultura de gestão de riscos, quais sejam: a construção da Ficha de Notificação de Eventos Adversos e da ficha de registro de Avaliação Diária do Risco de Queda; capacitação dos profissionais quanto à aplicação dos protocolos; capacitação da equipe de enfermagem quanto à utilização da Escala de Queda de Morse; e capacitação dos profissionais sobre suicídio e quanto ao manejo do paciente com ideações suicidas.
