predictors of the presence of the disease and investigate the postural balance of
elderly people with and without type 2 diabetes mellitus (DM2) and verify the
associated clinical characteristics. METHOD: This is an analytical observational
cross-sectional study, whose sample was selected by convenience and consisted
of elderly people aged 60 years or older, with and without a diagnosis of type 2
Diabetes Mellitus, according to the criteria of the American Diabetes Association
(ADA) , of both sexes, divided into two groups: group 1 (G1) - elderly diagnosed
with type 2 DM and group 2 (G2) - elderly without type 2 DM. The associations
between the total scores of G1 and G2 were verified by using the Mann-Whitney
test and the Chi-Square or Fisher's exact test. To obtain prognostic factors, a
multivariate logistic regression model was used and to obtain the final multivariate
model, variables with p< 0.10 in the univariate analysis were selected and the
process of variable selection Stepwise Forward Selection (stepwise) was applied.
For all statistical analyzes a significance level of 5% was adopted. RESULTS:
Significance was observed in the variables: BMI (p=0.048), number of
medications (p=0.001), motor TUGt (p=0.015), FPP (p<0.001), WHODAS
(p=0.011), EDG (p= 0.001), presence of foot changes (p<0.001), calluses
(p<0.001), joint changes (p<0.001), dermatological changes (p<0.001), vibratory
sensitivity (p<0.001), skin-protective sensitivity (p<0.001), orthostatic
hypotension (p=0.003) and fear of falls (p=0.002). Through the logistic regression
model with a “stepwise” selection process, the variables: number of medications,
number of diseases and EDG showed a significant association with DM2. Aging
and DM2 are important factors for the presence of psycho-cognitive and
functional deficits and this is often associated with impaired quality of life for these
individuals. Older people with DM2 are twice as likely to have depressive
disorders, on the other hand, comorbidity worsens the prognosis of both
diseases, leads to functional losses, higher rates of depression and cognitive problems. CONCLUSION: The elderly in G1 have greater impairment of postural
balance when associated with the presence of depressive symptoms, decreased
mobility, presence of comorbidities, polypharmacy, foot changes and fear of falls.
Predictive factors for the presence of the disease in the elderly are the greater
number of medications in use, the greater number of diagnosed diseases and the
presence of depressive symptoms. |

OBJETIVO: O Objetivo geral do estudo foi comparar os idosos com e sem Diabetes Mellitus tipo 2 (DM2) quanto aos fatores sociodemográficos, clínicos, funcionais e psico-cognitivos relacionados ao equilíbrio postural e a funcionalidade e os objetivos específicos foram: Comparar os fatores biopsicossociais entre idosos com e sem diabetes mellitus tipo 2 e identificar quais são os preditores da presença da doença e Investigar o equilíbrio postural das pessoas idosas com e sem Diabetes Mellitus tipo 2 (DM2) e verificar as características clínicas associadas. MÉTODO: Trata-se de um estudo observacional analítico de caráter transversal, cuja amostra foi selecionada por conveniência e constituída por idosos com 60 anos ou mais, com e sem diagnóstico de Diabetes Mellitus tipo 2, segundo os critérios da American Diabetes Association (ADA), de ambos os sexos, dividida em dois grupos: grupo 1 (G1) – idosos com diagnóstico de DM tipo 2 e grupo 2 (G2) - idosos sem DM tipo 2. As associações entre os escores totais do G1 e G2 foram verificadas por meio dos testes de Mann-Whitney e teste Qui- Quadrado ou Exato de Fisher. Para a obtenção de fatores prognósticos foi utilizado modelo de regressão logístico multivariado e para a obtenção do modelo multivariado final foram selecionadas as variáveis que apresentaram p< 0,10 na análise univariada e aplicado o processo de seleção de variáveis Stepwise Forward Selection (stepwise). Para todas as análises estatísticas foi adotado um nível de significância de 5%. RESULTADOS: Foi observado significância nas variáveis: IMC (p=0,048), número de medicamentos (p=0,001), TUG motora (p=0,015), FPP (p<0,001), WHODAS (p=0,011), EDG (p=0,001), presença de alterações nos pés (p<0,001), calosidade (p<0,001), alterações articulares (p<0,001), alterações dermatológicas (p<0,001), sensibilidade vibratória (p<0,001), sensibilidade cutâneo-protetora (p<0,001), hipotensão ortostática (p=0,003) e medo de quedas (p=0,002). Através do modelo de regressão logística com processo de seleção “stepwise” as variáveis: número de medicamentos, número de doenças e EDG apresentaram associação significante com DM2. O envelhecimento e o DM2 são importantes fatores para a presença de déficits psico-cognitivos e funcionais e isto, muitas vezes, está associado ao prejuízo da qualidade de vida destes indivíduos. Pessoas mais velhas com DM2 tem o dobro de chance de apresentarem transtornos depressivos, por outro lado, a comorbidade piora o prognóstico de ambas as doenças, leva às perdas funcionais, maiores taxas de depressão e problemas cognitivos. CONCLUSÃO: Os idosos do G1 apresentam maior prejuízo do equilíbrio postural, quando associados à presença de sintomas depressivos, diminuição da mobilidade, presença de comorbidades, polifarmácia, alterações nos pés e medo de quedas.
