Introduction: Outpatient nursing care for individuals with coronary artery disease and
undergoing percutaneous transluminal coronary angioplasty, lacks an assistance
structure for self-management. Self-management of the risks and chronic symptoms
of the disease by the individual and the achievement of healthy behaviors provide
better results in the therapeutic process, such as mitigating complications and
acquiring well-being and quality of life. Objective: To develop an outpatient nursing
care model for self-management of individuals undergoing percutaneous transluminal
coronary angioplasty. Methodological path: This is a methodological research with a
qualitative approach carried out through the deductive-inductive logical reasoning
process and based on the Systems Model of Betty Neuman and The Individual and
Family Self-Management Theory. The study was developed in three interrelated
moments, namely: The first moment corresponded to the selection of concepts, based
on the referential, and the integrative literature review, which identified the strategies
used by nurses and family in caring for the individual in post - coronary angioplasty to
support self-management. The second moment was conducted at the Hospital
Universitário Alcides Carneiro, located in Campina Grande / PB, Brazil, through
interviews with nurses, individuals in post-coronary angioplasty and family members.
The data from the field research were analyzed using content analysis. In the third
moment, the results of the previous moments were compared, analyzed and integrated
in the light of the theoretical framework, which allowed the elaboration of a graphic
representation of the care model. The study obtained a favorable opinion from the
Research Ethics Committee under number 4,630,775 and CAAE
43329321.8.0000.5182. Results: The research developed the “Outpatient Nursing
Care Model for Self-Management of the Individual in the Late Postpartum of
Percutaneous Transluminal Coronary Angioplasty”, based on theoretical nursing
concepts, integrative review and field research. The central strategies and concepts
identified in support of self-management are: educational support, emotional support
and social support network. Nurses, individuals and their families understood that the
following aspects are essential in nursing care for self-management: control of
cardiovascular risk factors, family support, outpatient nursing consultation, medication
adherence, self-efficacy and belief, dialogue, reduction of anxiety/ stress and
management support. Final considerations: The nursing care model from the
perspective of self-management developed in this study offers a guiding and strategic
theoretical framework for assistance to people with coronary artery disease undergoing
angioplasty and represents a contribution to the knowledge of nursing in secondary
prevention in health . Thus, it is believed that the present study contributes to the
advancement of nursing knowledge and the direction of care provided by this
professional to post-angioplasty patients. |

Introdução: O cuidado de enfermagem ambulatorial ao indivíduo submetido à angioplastia coronária transluminal percutânea necessita de uma estrutura assistencial para a autogestão do cuidado que sirva como modelo para a implementação do cuidado a essa população. O autogerenciamento desse indivíduo proporciona resultados melhores no processo terapêutico, como mitigação de complicações e a aquisição do bem-estar e qualidade de vida. Objetivo: Desenvolver um modelo de cuidado de enfermagem ambulatorial para autogestão do indivíduo submetido à angioplastia coronária transluminal percutânea. Caminho metodológico: Trata-se de uma pesquisa metodológica, teórica, com abordagem qualitativa realizada por meio do processo de raciocínio lógico dedutivo-indutivo e alicerçada no Modelo de Sistemas de Betty Neuman e na The Individual and Family Self-Management Theory. O estudo foi desenvolvido em três momentos, a saber: o primeiro correspondeu à seleção dos conceitos, fundamentada no referencial, e à revisão integrativa da literatura, a qual identificou as estratégias utilizadas pelos enfermeiros no cuidado ao indivíduo em pós-angioplastia coronária para o apoio à autogestão; o segundo foi conduzido no Hospital Universitário Alcides Carneiro, localizado em Campina Grande, Paraíba, por meio de entrevistas com enfermeiros, indivíduos em pós-angioplastia coronária e seus familiares; no terceiro momento, os resultados anteriores foram analisados e integrados sob a luz do referencial teórico, o que permitiu a elaboração de uma representação gráfica do modelo de cuidado que culminou no desenvolvimento de uma estrutura conceitual em forma de modelo. O estudo obteve parecer favorável do Comitê de Ética em Pesquisa sob número 4.630.775 e CAAE 43329321.8.0000.5182. Foram obedecidas as recomendações da Resolução n° 466/12, aprovada pelo Conselho Nacional de Saúde do Ministério da Saúde, em 12 de dezembro de 2012. Resultados: A pesquisa desenvolveu o “Modelo de Cuidado de Enfermagem Ambulatorial para Autogestão do indivíduo em pós tardio de angioplastia coronária transluminal percutânea”, embasado nos conceitos teóricos de enfermagem, revisão integrativa e pesquisa de campo. As estratégias e conceitos centrais identificados no apoio à autogestão são: apoio educacional, suporte emocional e rede de apoio social. Os enfermeiros, indivíduos e seus familiares compreenderam que os seguintes aspectos são essenciais no cuidado de enfermagem para autogestão: controle dos fatores de risco cardiovasculares, apoio da família, consulta de enfermagem ambulatorial, adesão medicamentosa, autoeficácia e crença, diálogo, redução da ansiedade/ estresse e o suporte da gestão. Considerações finais: O modelo de cuidado de enfermagem na perspectiva de autogestão desenvolvido no presente estudo oferece uma estrutura teórica norteadora e estratégica da assistência às pessoas com doença da artéria coronária submetida à angioplastia e representa uma contribuição para o conhecimento da enfermagem na prevenção secundária em saúde.
