CNS damage associated with vestibular symptoms occurs due to lesions in the central vestibular pathways. Objective: (1) To review the scientific literature on the applicability and results of the Video Head Impulse Test (vHIT) in diseases of the CNS. (2) To characterize the neurological and vestibular findings of clinical cases with Multiple Sclerosis (MS). Method: Dissertation structured in two articles. The first (1) is an integrative review of 18 scientific publications on the use of vHIT in individuals with CNS disease. The second (2) is the description of three clinical cases with MS. Results: (1) As for vHIT, the gain of the vestibulo-ocular reflex (VOR) was reduced in most studies, with the occurrence of compensatory saccades and described changes in oculomotor tests, such as the presence of spontaneous and semi-spontaneous nystagmus, in the visual VOR and VOR supression, in the saccade test and skew deviation test. (2) Vestibular symptoms and alterations were observed in at least one of the clinical tests of body balance and cerebellar function. In vHIT, alterations were obtained in oculomotor tests, such as the presence of semi-spontaneous nystagmus and in the latency and velocity parameters of the saccade test, and reduced gain in one or more vertical channels. The association of these findings suggests the presence of central vestibular dysfunction, compatible with the lesions detected in imaging exams. Conclusion: The findings of both studies provided evidence of changes in vestibular function and body balance in different neurological conditions.

Introdução: As doenças do sistema nervoso central (SNC) acometem o encéfalo e a medula espinhal. Os danos ao SNC associados aos sintomas vestibulares ocorrem devido lesões nas vias vestibulares centrais. Objetivo: (1) Revisar a literatura científica sobre a aplicabilidade e resultados do Video Head Impulse Test (vHIT) em doenças do SNC. (2) Caracterizar os achados neurológicos e vestibulares de casos clínicos com Esclerose Múltipla (EM). Método: Dissertação estruturada em dois artigos. O primeiro (1) trata-se de uma revisão integrativa de 18 publicações científicas sobre o uso do vHIT em indivíduos com doença do SNC. O segundo (2) trata-se da descrição de três casos clínicos com EM. Resultados: (1) Quanto ao vHIT, o ganho do reflexo vestíbulo-ocular (RVO) foi reduzido na maioria dos estudos, com ocorrência de sacadas compensatórias e descritas alterações em testes oculomotores, como presença de nistagmo espontâneo e semi-espontâneo, no RVO com e sem otimização visual, no teste sacádico e de desvio de inclinação. (2) Observou-se presença de sintomas vestibulares e alterações em pelo menos uma das provas clínicas do equilíbrio corporal e função cerebelar. No vHIT, obteve-se alterações em testes oculomotores, como presença de nistagmo semi-espontâneo e nos parâmetros latência e velocidade do teste sacádico, e ganho reduzido em um ou mais canais verticais. A associação destes achados sugere a presença de disfunção vestibular central, compatível com as lesões detectadas nos exames de imagem.
