Gram staining and biochemical tests identified the strain as belonging to the Acinetobacter baumanni complex, but the MALDI-TOF system identified as A. radioresistens. Antimicrobial susceptibility testing and blaOXA-23 gene detection were performed using diffusion and PCR techniques, respectively. The strain was sensitive to all antibiotics tested, including carbapenems, despite bearing the blaOXA23 gene. Acinetobacter radioresistens can be considered an important agent of opportunistic infections in immunocompromised patients. Este é a descrição do primeiro caso de bacteremia adquirida na comunidade causada por Acinetobacter radioresistens no Brasil. Cultura pura em ágar MacConkey foi obtida a partir do sangue de um paciente, masculino, 73 anos de idade, com doença de Alzheimer e de Parkinson. A coloração de Gram e provas bioquímicas identificaram a cepa como pertencente ao complexo Acinetobacter baumanni, porém o sistema MALDI-TOF identificou como A. radioresistens. Testes de susceptibilidade aos antimicrobianos e detecção do gene blaOXA-23 foram realizados, usando as técnicas do disco difusão e PCR, respectivamente. A cepa apresentou-se sensível a todos os antibióticos testados, incluindo os carbapenems, apesar de portar o gene blaOXA23.
