description Introduction: The oral health condition of elderly can be influenced by several factors, especially in the socioeconomic-demographic context. Aim: to identify changes in the oral health condition of the elderly over time, based on the application of a multidimensional indicator. Methods: this is an observational and longitudinal study, which was performed in Macaíba / RN. For that, 176 elderly individuals were assessed in 2010/2011 and five years later, using a multidimensional indicator constructed through a factorial analysis with eleven variables. The factors produced had their scores added and categorized from the median, at two levels (favorable and unfavorable oral health). For data analysis, the Student's t-test was used for dependent samples, Chi-Square test and Relative Risk (95% CI). Results: After the factorial analysis, four factors were extracted (teeth present and periodontally impaired, caries attack, household density and age and schooling). The change in oral health condition over time was significant (p <0.001) and represented a worsening of the oral health condition in 18.6% (CI 95%: 12.9 - 24.3) of the sample. Among the factors that compose the indicator, those most related to the change in the oral health condition of these elderly people were caries attack, household density and age. There was no significance between the independent variables of the study and the change in oral health status over time. Conclusion: Few older people have worsened oral health status over time, which such worsening was due to the insufficient access and changes in the family structure of the elderly, reinforcing the importance of the multifactorial indicator as a tool for the of public policies planning targeted to this population.

Introdução: a condição de saúde bucal da pessoa idosa pode ser influenciada por diversos fatores, especialmente no âmbito socioeconômico-demográfico. Objetivo: identificar alterações na condição de saúde bucal de idosos ao longo do tempo, a partir da aplicação de um indicador multidimensional. Metodologia: trata-se de um estudo individuado, observacional e longitudinal, realizado em Macaíba/RN. Para tanto, 176 idosos foram avaliados em 2010/2011 e cinco anos após, a partir de um indicador multidimensional construído através da analise fatorial com onze variáveis. Os fatores foram produzidos tiveram seus escores somados e categorizados a partir da mediana, em dois níveis (saúde bucal favorável e desfavorável). Para análise dos dados, foram utilizados o teste t de Student para amostras dependentes, o teste do Qui-Quadrado de Pearson e o Risco Relativo (IC 95%). Resultado: Após a análise fatorial, quatro fatores foram extraídos (Dentes presentes e comprometidos periodontalmente; ataque de cárie; densidade domiciliar e idade e escolarização). A alteração da condição de saúde bucal ao longo do tempo foi significativa (p<0,001), e representou a piora da condição de saúde bucal em 18,6% (IC 95%: 12,9 – 24,3) da amostra. Dentre os fatores que compuseram o indicador, aqueles mais relacionados à mudança da condição de saúde bucal desses idosos foram o ataque de cárie, densidade domiciliar e idade. Não houve significância para a relação das variáveis independentes do estudo e a mudança na condição de saúde bucal ao longo do tempo.
