The aim of the study was to evaluate the main factors that may interfere with warfarin anticoagulant therapy, using the Therapeutic Time (TTR), as determined by the sequence of results of the International Normalized Ratio (INR), calculated by the Rosendal method. A total of 113 patients treated at the Outpatient Anticoagulation Clinic of the Onofre Lopes University Hospital (HUOL), from 2015 to 2019, were considered. According to the results, the majority of patients were female (74.34%) and the use of heart valve and biologic metal are highlighted as the most common indications for anticoagulant use (35.40%). The average of prescription drugs was 6.21 per patient. The mean TTR value was 41%, with 15 patients with moderate anticoagulation control (TTR 60-75%), 8 high quality detached patients (TTR> 75%) and 90 patients presenting in the group with TTR below recommended (TTR <60% ). Polypharmacy, drug interactions, antiphospholipid syndrome (APS), hypothyroidism and doses <35mg / week were the factors that contributed to the lower treatment effectiveness associated with TTR values below 60%. It concludes that most patients are at risk of hemorrhagic or thrombotic complications, and it is necessary to create new strategies that promote better effectiveness and safety in the treatment of patients.

A varfarina é considerada um importante anticoagulante oral e vem sendo utilizada por mais de 50 anos. É considerado uns dos 10 fármacos mais envolvidos em reações adversas devido a sua estreita janela terapêutica, extensa lista de interações medicamentosas e alimentares, expressando variabilidade de dose-resposta de paciente para paciente. O estudo teve como objetivo avaliar os principais fatores que podem interferir na terapêutica anticoagulante da varfarina, utilizando como parâmetro de efetividade o Time in Terapeutic Range (TTR), determinado pela sequência de resultados da Relação Normalizada Internacional (RNI), calculado pelo método de Rosendaal. Foram considerados no estudo 113 pacientes atendidos no ambulatório de anticoagulação do Hospital Universitário Onofre Lopes (HUOL), durante os anos de 2015 a 2019. De acordo com os resultados, a maioria dos pacientes eram do sexo feminino (74,34%) e o uso de válvula cardíaca metálica e biológica se destacaram como as indicações mais comuns para o uso do anticoagulante (35,40%). A média de medicamentos prescritos foi de 6,21 por paciente. O valor médio de TTR foi de 41%, sendo que 15 pacientes tiveram controle moderado da anticoagulação (TTR 60-75%), 8 pacientes se destacaram com alta qualidade no tratamento (TTR >75%) e 90 pacientes se enquadraram no grupo com valores de TTR abaixo do recomendado (TTR<60%). A polifarmácia, interações medicamentosas, Síndrome Antifosfolípide (SAF), hipotireoidismo e doses <35mg/semana foram os fatores que contribuíram para a menor efetividade do tratamento associado aos valores de TTR inferiores a 60%.
