that are increasingly observed in university students. In the last decade in Brazil, there was a
process of migration of students to a university city due to the growth of higher education in
less urbanized cities, which can generate changes in lifestyle and in the sleep habits of these
students. The study of how the social environment can impact on the health of the individuals
can be carried out based on models of social determinants of health, which can be of a structural
or intermediate nature. The first is related to socioeconomic positions, education and work
issues and the second is related to behavioral influences at the individual level. In view of this,
the aim of this study was to analyze which factors of the Social Determinants of Health are
associated with the duration of sleep and social lag in university students in the country city of
Rio Grande do Norte (RN). METHODS: Developed with university students from the Faculty
of Health Sciences of Trairi, located in Santa Cruz-RN, through the application of two
questionnaires: “Health and sleep” and the “Munich chronotype questionnaire”. To analyze the
association between the independent variables and the sleep duration outcomes at week and
weekend, and social lag, multiple linear regression was adopted with stepwise forward method.
The level of significance adopted was 5%. RESULTS: There was no association between the
determinants of health (socioeconomic class, work, knowledge about sleep and the start time of
classes) with variables sleep duration in the week and weekend, and social lag. Regarding the
intermediate determinants of health, the predictive variables for the duration of sleep during the
week were chronotype (β = -0.55, p = 0.001), gender (β = - 24.78, p = 0.008), leisure activities
(β = -27.94; p = 0.003), sleeping place (β = -42.78; p = 0.005) and housing (β = 24.95; p =
0.010). For the duration of sleep over the weekend were gender (β = -36.36; p = 0.001),
chronotype (β = 0.24; p = 0.001), sleeping place (β = -58.15; p = 0.001), recreational and
religious activities (β = -31.11; p = 0.027), leisure activities (β = -25.10; p = 0.021) and
academic demand (β = -23.60; p = 0.025). The predictive variables of social lag were leisure
activities (β = 23.24), academic demand (β = 19.51), household chores (β = 17.61) and
stimulating drinks (β = -15.17). In addition, it was observed that 43% of the sample had a sleep
duration in the week less than 6h (average = 6h39min ± 1h28min), while 92.3% had sleep
duration of more than 6h (average = 8h01min ± 1h32min). The average social lag was 1h21min
± 55min. 34.2% of the participants had social lag less than 1h, 52.3% had social lag between
1h and 2h and 13.4% had social lag greater than 2h. CONCLUSION: The observed associations
were related to the intermediate determinants of health classified as biological, behavioral and
housing conditions of university students. University students from the interior of RN had
shorter sleep duration on school days and longer duration on free days, and 65.7% of the sample
had social lag greater than 1h. This study carried out with students from the country city of the
state of Rio Grande do Norte is a pioneer study and may contribute to the discussions related to
the university context. In addition, considering that these university students will be future
health professionals, the results can serve as a basis for discussing the impact of social
determinants of health on sleep/wake cycle of individuals and discussing sleep hygiene in the
public health context. |

INTRODUÇÃO: A irregularidade nos horários de dormir e acordar entre os dias livres e de trabalho/estudo, denominada de social lag, somada a redução nas horas de sono na semana são hábitos cada vez mais observados nos universitários. Na última década no Brasil, ocorreu um processo migração de estudantes para cidades universitárias decorrente da interiorização do ensino superior, o que pode gerar mudanças no estilo de vida e nos hábitos de sono dos estudantes. O estudo de como o meio social pode impactar na saúde dos indivíduos pode ser realizado a partir dos modelos de determinantes sociais da saúde, os quais podem ser de caráter estruturais ou intermediários. O primeiro relacionado a questões socioeconômicas, educacionais e de trabalho e o segundo ligado às influências comportamentais de nível individual. Diante disto, o objetivo desse estudo foi analisar quais fatores dos Determinantes sociais de Saúde estão associados com a duração de sono e social lag em universitários no interior do Rio Grande do Norte (RN). MÉTODOS: Desenvolvido com estudantes da Faculdade de Ciências da Saúde do Trairi, localizada em Santa Cruz-RN, através da aplicação de 02 questionários: “A saúde e o sono” e o questionário de cronotipo de Munique. Para analisar a associação entre as variáveis independente e os desfechos duração do sono na semana e fim de semana, e social lag foi adotada a regressão linear múltipla com método stepwise forward. O nível de significância adotado foi de 5%. RESULTADOS: Não houve associação entre os determinantes estruturais de saúde (classe socioeconômica, trabalho, conhecimento sobre o sono e horário de início das aulas) e as variáveis desfecho duração de sono na semana e fim de semana, e o social lag. Com relação aos determinantes intermediários da saúde as variáveis preditoras para a duração de sono na semana foram: cronotipo (β = -0,55, p= 0,001), sexo (β = - 24,78, p=0,008), atividades de lazer (β = -27,94; p=0,003), local de dormir (β = -42,78; p=0,005) e moradia (β = 24,95; p=0,010). As variáveis preditoras da duração de sono no fim de semana foram: sexo (β = -36,36; p=0,001), cronotipo (β = 0,24; p=0,001), local de dormir (β = -58,15; p=0,001), atividades recreativas e religiosas (β = -31,11; p=0,027), atividades de lazer (β = -25,10; p=0,021) e demanda acadêmica (β = -23,60; p=0,025). As variáveis preditoras do Social lag foram: atividades de lazer (β =23,24), demanda acadêmica (β =19,51), afazeres domésticos (β = 17,61) e bebidas estimulantes (β =-15,17). Além disso, foi observado que 43% da amostra apresentou duração de sono na semana menor que 6h (média = 6h39min ± 1h28min), enquanto 92,3% apresentaram duração de sono maior que 6h no fim de semana (média = 8h01min ± 1h32min). O social lag médio observado foi de 1h21min ± 55min. 34,2% dos participantes apresentaram social lag menor que 1h, 52,3% exibiram social lag entre 1h e 2 h e 13,4% obtiveram social lag maior que 2h. CONCLUSÃO: As associações observadas foram referentes aos determinantes intermediários da saúde classificados como fatores biológicos, comportamentais e de condições de moradia dos universitários. Os universitários do interior do RN apresentaram menor duração de sono nos dias de aula e maior duração nos dias livres, e 65,7% da amostra apresentou social lag maior que 1h. Este estudo realizado com estudantes do interior do Rio Grande do Norte é pioneiro e poderá contribuir para as discussões relativas ao contexto universitário.
