The use of Hypodermoclysis as subcutaneous therapy presents itself as an effective technique
for medication and fluid replacement, when the oral and intravenous routes are not viable in
any way. Especially in the fields of geriatrics and oncology, due to the clinical conditions that
patients have. Therefore, it is necessary to develop and implement instruments to standardize
care in relation to its use, from its choice to the technique, in an attempt to support the decisionmaking of health professionals, and thus, foster care in a planned. In this perspective, this study
establishes the following guiding question: What elements are necessary to develop and validate
content and appearance of an algorithm that guides the eligibility of adult patients to use
hypodermoclysis? It aims to develop and validate content and appearance of an algorithm to
guide the eligibility of hypodermoclysis in an adult patient. This is a methodological study for
the construction and validation of the content and appearance of an assistance tool. The research
considers the proposal of the Pasquali Model, which is organized in three stages, namely:
theoretical procedures, in which a scope review was carried out to identify and map the evidence
on the eligibility / indication criteria of adult patients for use hypodermoclysis; empirical
procedures, which took place through the construction of the algorithm, which was developed
using Bizagi Modeler software, for content and appearance validation via Google Forms and
the Delphi steps with expert judges in the area to obtain consensus; and, analytical procedures
in which the analysis of the data from the previous phase was carried out using the content
validation coefficient. Items with a minimum agreement of 80% between judges were
considered valid. The study was approved by the Research Ethics Committee. After building
the algorithm, Delphi I and II steps were performed to validate the content and appearance,
based on Pasquali's criteria: behavior, objectivity, simplicity, clarity, relevance, precision,
variety, modality, typicality, credibility, breadth and balance. The final round of validation
consisted of eight judges, including six women and two men, with an average age of 37.7 years.
Three of these judges work mainly in teaching, three conciliate teaching and assistance, and
two exercise their activities only in assistance. So, the time that experts work with the theme
ranged from 6 to 20 years. The level of agreement between the judges was calculated and the
content validation reached a total value of 0.95, while the appearance was 0.93. From this, it is
concluded that the contributions of the experts were paramount, since they provided the
construction of an instrument appropriate to the reality of assistance, with the use of more
coherent flows and terms. In addition, the patient's eligibility algorithm for hypodermoclysis
produced and validated in this study, will contribute to safer nursing care, as well as to the
valorization of the work provided by nurses, as it allows a continuum care process. |

O uso da Hipodermóclise como terapia subcutânea se apresenta como uma técnica eficaz para medicação e reposição de fluidos, quando as vias oral e endovenosa estão inviáveis de alguma forma. Especialmente nos campos da geriatria e oncologia, devido as condições clínicas que os pacientes apresentam. Logo, se faz necessária a elaboração e a implementação de instrumentos de padronização do cuidado em relação ao seu uso, desde sua escolha até a técnica, na tentativa de apoiar a tomada de decisão dos profissionais de saúde, e assim, fomentar o cuidado de forma planejada. Nesta perspectiva, esse estudo estabelece a seguinte questão norteadora: Quais elementos são necessários para desenvolver e validar conteúdo e aparência de um algoritmo que oriente a elegibilidade de pacientes adultos ao uso da hipodermóclise? Tem como objetivo desenvolver e validar conteúdo e aparência de um algoritmo para orientar na elegibilidade da hipodermóclise em paciente adulto. Trata-se de um estudo metodológico para construção e validação de conteúdo e aparência de uma ferramenta assistencial. A pesquisa considera a proposta do Modelo de Pasquali, o qual é organizado em três etapas, a saber: procedimentos teóricos, em que uma revisão de escopo foi realizada para identificar e mapear as evidências sobre os critérios de elegibilidade/indicação de pacientes adultos para uso da hipodermóclise; procedimentos empíricos, que se deu pela construção do algoritmo, que foi elaborado a partir do software Bizagi Modeler, para validação do conteúdo e aparência via Google Forms e as etapas Delphi com juízes especialistas na área para obtenção de consenso; e, procedimentos analíticos em que foi realizada a análise dos dados da fase anterior por meio do coeficiente de validação de conteúdo. Foram considerados válidos os itens com concordância mínima de 80% entre os juízes. O estudo foi aprovado por comitê de Ética em pesquisa. Após a construção do algoritmo, realizou-se as etapas Delphi I e II para validação do conteúdo e aparência, a partir dos critérios de Pasquali: comportamento, objetividade, simplicidade, clareza, relevância, precisão, variedade, modalidade, tipicidade, credibilidade, amplitude e equilíbrio. A rodada final de validação foi composta por oito juízes, dentre eles seis mulheres e dois homens, com idade média de 37,7 anos. Três desses juízes atuam majoritariamente na docência, três concilia docência e assistência, e dois exercem suas atividades apenas na assistência. De modo que, o tempo que os experts trabalham com a temática variou de 6 a 20 anos. Foi calculado o nível de concordância entre os juízes e a validação do conteúdo atingiu o valor total de 0,95, enquanto a aparência foi de 0,93. A partir disso, conclui-se que as contribuições dos expertos foram primordiais, uma vez que proporcionaram a construção de um instrumento adequado a realidade da assistência, com o emprego de fluxos e termos mais coerentes.
