This study aims to know the perception of adolescents accompanied by the ESF under mental health service. This is a study of qualitative approach and descriptive-propositional nature, the existential action research type, which used the projective interview technique, using Sandplay for data collection. The interviews were developed with the help of a semi-structured script, which was organized in four question sessions, which had the following questions: What is adolescence for you?; What causes mental distress at this stage of life?; Is there a family relationship with this suffering?; and, Do you feel welcomed by the health team?. Along the interview was develop the technique called Sandplay or Sand Game. As a result, it was possible to understand how the adolescent accompanied by the ESF uses the municipality`s mental health service and its implications on the care received. Through projective technique exposes thoughts, sometimes not expressed in the interview. In this sense, the Family Health Strategy must be able to develop new care modalities based on the real problems of the clientele in question, thus contributing to the planning and organization of care, as well as to the adolescent mental health care network. The study obtained a favorable opinion from the Research Ethics Committee HUOL/UFRN (Parecer n. 3.440.688 e CAAE: 13447719.9.0000.5292). A adolescência é um período conhecido por mudanças e vulnerabilidade, expondo o adolescente a situações de violência, adoecimento mental, sendo necessário manter-se atento as necessidades desse grupo. Este estudo tem por objetivo conhecer a percepção do adolescente acompanhado pela ESF sob o serviço de saúde mental. Trata-se de um estudo de abordagem qualitativa e natureza descritiva-propositiva, do tipo pesquisa-ação existencial, que utilizou a técnica de entrevista projetiva, com uso do Sandplay para coleta de dados. As entrevistas foram desenvolvidas com auxílio de um roteiro semiestruturado, que encontrava-se organizado em quatro sessões de perguntas, que contava com as seguintes questões: O que é adolescência para você?; O que causa sofrimento mental nesta fase da vida?; Existe relação da família com esse sofrimento? e, você se sente acolhido pela equipe de saúde?”. Junto a entrevista foi desenvolvida a técnica denominada de Sandplay ou Jogo de areia. Como resultado, foi possível compreender como o adolescente acompanhado pela ESF utiliza o serviço de saúde mental do município e suas implicações sobre a assistência recebida. Através da técnica projetiva expõe pensamentos, por vezes, não expressos na entrevista. Nesse sentido, a Estratégia Saúde da Família deve ser capaz de desenvolver novas modalidades assistenciais com base nos reais problemas da clientela em questão contribuindo assim para o planejamento e organização do cuidado, bem como, da rede de atenção à saúde mental do adolescente.
