Hospital’s purchasing management deals with different item categories. These
categories, due to their nature of supply, demand and technological knowledge, affect
buyer’s rationality. Considering the criteria and strategies for purchasing intrinsic to
hospital’s context, the present study aims to propose a hospital purchasing portfolio
model to classify purchasing items. Conflicting interests within hospital supply chain
actors are discussed based on Agency Theory. In the methodological aspect, the study
uses qualitative and quantitative approaches. A systematic literature review (RSL) was
performed to identify the criteria and strategies used in hospital’s purchasing decision
process. The criteria, derived from RSL, were assessed through an online survey with
39 purchasing managers from large private hospitals in the southern, southeastern and
northeastern regions of Brazil. Data were subjected to principal component analysis
(PCA) validating eleven criteria (lead-time, cost, reputation, negotiation, monopoly,
volume, customization, storage, assistance, physician’s preference and knowledge).
The latent variables from PCA formed the dimensions of the proposed model: universal
purchasing parameters; dependence on supply; and knowledge asymmetry. The
model was applied in a large private hospital, analyzing a sample of twelve hospital
purchasing items. On the item classification step, the Fuzzy-TOPSIS method was
used. Of the twelve items in the sample, two were classified in the “non-critical”
category, two in “leverage”, two in “bottleneck”, three in “strategic” and three in “critical”.
The result was validated by hospital purchasing managers. A sensitivity analysis
attested the robustness of the model. |

Tais categorias, em função da sua natureza de oferta, de demanda e de conhecimento tecnológico, afetam a racionalidade do comprador. Considerando os critérios e as estratégias de compras intrínsecas ao contexto hospitalar, o presente estudo tem por objetivo propor um modelo de portfólio de compras hospitalares para classificação dos itens comprados. Os interesses conflitantes entre os atores da cadeia de suprimentos hospitalar são discutidos à luz da Teoria da Agência. No aspecto metodológico, o estudo utiliza abordagem qualitativa e quantitativa. Uma revisão sistemática de literatura (RSL) foi efetivada para identificar os critérios e estratégias, utilizados no processo decisório de compras hospitalares. Os critérios, oriundos da RSL, foram avaliados por meio de uma survey on-line com 39 gestores de compras de hospitais particulares de grande porte, pertencentes às regiões Sul, Sudeste e Nordeste do Brasil. Os dados foram submetidos à análise de componentes principais (PCA) validando onze critérios (tempo, custo, reputação, negociação, monopólio, volume, customização, armazenagem, assistência, preferência médica e conhecimento). As variáveis latentes, oriundas da PCA, formaram as dimensões do modelo proposto: parâmetros universais de compras; dependência no fornecimento; e assimetria de conhecimento. O modelo foi aplicado em um hospital privado de grande porte, analisando uma amostra de doze itens de compras hospitalares. Na etapa de classificação dos itens, foi utilizado o método Fuzzy-TOPSIS. Dos doze itens da amostra, dois foram classificados na categoria “não-crítico”, dois na “alavancagem”, dois na “gargalo”, três na “estratégico” e três na “crítico”. O resultado foi validado pelos gestores de compras do hospital.
