Microcephaly is a congenital malformation characterized by a reduction of the cephalic
perimeter in at least two standard deviations of the expected mean for age and sex, presenting
motor and cognitive alterations that vary according to the degree of cerebral involvement. It is
considered that a home environment rich in opportunities for motor stimulation has a positive
impact on the motor and cognitive performance of children who are early exposed to these
stimuli. The objective was to evaluate motor affordances in the home environment of children
with microcephaly and their cognitive performance. This was an exploratory-descriptive
study, with a cross-sectional, quantitative approach, carried out at the Child Rehabilitation
Center and the Clinical School of Physiotherapy. The sample consisted of 30 children with
microcephaly, between 18 and 42 months of age, and their families. The Affordances in the
Home Environment for Motor Development (AHEMD) and the cognitive scale of the Bayley
Scales of Infant and Toddler Development III (BSITD-III) were used as instruments of data
collection. The study was approved by the Research Ethics Committee under opinion No.
2,765,621. It was observed that there were the same percentage of male and female children
and that 50% had a Z score of head circumference of -3 at birth, with a mean gestational age
of 38.44 weeks and the mean birth weight of 2,798.67 grams. The majority (56.67%)
underwent therapy up to twice weekly. Considering characteristics of the family, it was
observed that there were up to two adults (70.00%), with half having only one child and the
other half presenting two or more children in the majority of the residences. Most of the
children (96.67%) have never been attended nursery or school and lived in a house (96.67%).
The fathers and mothers had scholar education between fifth and eighth grade, with 51.72%
and 46.67%, respectively. Most households had a family income above a minimum wage
(56.67%), with socioeconomic status D. Regarding motor affordances in the home
environment, the majority of households had a weak physical income (53.33%) and very good
interior (70.00%), stimulation variety with very low yield (50.00%) and toys and materials in
the very weak housing (100.00%). Therefore, the majority of the children (73.33%) lived in
homes with low motor affordances, with a mean of 8.47 (± 1.72) points in the total
standardized AHEMD score. Most children presented extremely low cognitive performance
(96.67%). As a research product, a guide for parents and family with guidelines on
stimulating and expanding affordances in the home environment has been developed to help
children reach their full potential. Therefore, it was concluded that the majority of children
with microcephaly were from families with socioeconomic vulnerability, presented few
opportunities for motor stimulation in their homes and severe cognitive impairment. In
addition, it was suggested the promotion of a home environment rich in motor affordances,
since a stimulating environment seems to favor children's development. |

A microcefalia consiste em uma má formação congênita caracterizada por um perímetro cefálico inferior a dois desvios-padrão ou mais da média esperada para a idade e sexo, com presença de alterações motoras e cognitivas que variam de acordo com o grau de acometimento cerebral. Considera-se que um ambiente domiciliar rico em oportunidades de estimulação motora tem um impacto positivo no desempenho motor e cognitivo das crianças precocemente expostas a esses estímulos. Objetivou-se avaliar affordances motoras no ambiente domiciliar de crianças com microcefalia e o desempenho cognitivo destas. Tratouse de estudo exploratório-descritivo, com delineamento transversal, de abordagem quantitativa, realizado no Centro de Reabilitação Infantil e na Clínica Escola de Fisioterapia. A amostra foi composta por 30 crianças com microcefalia, entre 18 e 42 meses de idade, e suas famílias. Utilizou-se como instrumentos de coleta de dados o Affordances in the Home Environment for Motor Development (AHEMD) e a escala cognitiva das Bayley Scales of Infant and Toddler Development III (BSITD-III). Após a aprovação do estudo pelo Comitê de Ética em Pesquisa por meio do parecer n° 2.765.621. Observou-se que havia o mesmo percentual de crianças do sexo masculino e feminino e que 50% delas apresentavam escore Z de perímetro cefálico de -3 ao nascer, com média da idade gestacional de 38,44 semanas e a média de peso ao nascer de 2.798,67 gramas. A maior parte (56,67%) realizava terapia até duas vezes na semana. Sobre as características da família, observou-se que na maioria das residências existiam até dois adultos (70,00%), sendo que a metade tinha somente uma criança e a outra metade apresentava duas ou mais crianças. A maioria das crianças (96,67%) nunca frequentou creche ou escola e moravam em uma casa (96,67%). Os pais e mães tinha escolaridade entre quinta a oitava série, com 51,72% e 46,67%, respectivamente. A maior parte tinha rendimento familiar acima de um salário mínimo (56,67%), com classificação socioeconômica D. No que se refere às affordances motoras no ambiente domiciliar, a maioria das residências apresentou espaço físico exterior com rendimento fraco (53,33%) e o interior muito bom (70,00%), variedade de estimulação com rendimento muito fraco (50,00%) e brinquedos e materiais existentes na habitação muito fraco (100,00%). Portanto, a maioria das crianças (73,33%) vivia em lares com baixas affordances motoras, com média de 8,47 (±1,72) pontos no escore padronizado total do AHEMD. A maioria das crianças apresentou desempenho cognitivo extremamente baixo (96,67%). Como produto da pesquisa, foi elaborado um guia destinado aos pais e familiares com orientações sobre estimulação e ampliação de affordances no ambiente domiciliar a fim de que as crianças alcancem o máximo de seus potenciais. Logo, concluiu-se que a maioria das crianças com microcefalia era de famílias com vulnerabilidade socioeconômica, apresentou poucas oportunidades de estimulação motora em seus lares e severo comprometimento cognitivo.
