PIAFEX-SD: Programa de Intervenção em Autorregulação e Funções Executivas para a Síndrome de Down
description In recent years, research has highlighted that deficits in executive functions (EF) constitute one of the main characteristics of the cognitive functioning of individuals with Down Syndrome (DS). Difficulties are identified in the initiation of activities, construction of plans and strategies, updating and monitoring of these, maintenance of focus and persistence, cognitive flexibility, among other aspects. Such damages negatively impact the lives of these individuals and point to the need for the elaboration of intervention programs that seek to stimulate the development of EF in this clinical subgroup, since these functions are considered fundamental for the accomplishment of several activities of our daily life, especially for learning and school success. In this sense, the present research had as objective to adapt an intervention program of the EF to the clinical group of the DS. In order to achieve this objective, two studies were carried out, the study I: I) Investigation of aspects of the Neuropsychological Phenotype of school children with DS; and study II) PIAFEX-DS: Intervention Program in Self-Regulation and Executive Functions for DS. Thirty-one children diagnosed with DS, aged 7 to 12 years, of both sexes, as well as their respective parents or guardians participated in the study. These were assessed through specific neuropsychological scales and measures. The results of this study contributed to the understanding of aspects of the neuropsychological phenotype of DS, aspects that were taken into account in the adaptation of the intervention in study II, such as the pronounced deficits in working memory, the level of development lowered in the different subdomains of the adaptive behavior, the observed mental ages and the presence or absence of ADHD symptoms in these children and the impact of this for their results. In relation to study II, initially, a detailed analysis of the modules and activities that PIAFEX composite was performed, aiming to select them and adapt them to the neuropsychological phenotype of DS. After the adaptation, the program was sent to specialists judges in the DS, with the objective of evaluating it, as well as making possible its improvement and/or reformulation. After the synthesis of the judges' evaluation, the program was piloted with a group of three children with DS, in order to verify their suitability for this clinical group. At the end of this process, the program was reviewed and the last modifications were made. This research made possible the availability to the population with DS of an unprecedented intervention program aimed at the stimulation of EF. Such a study may serve as a basis for subsequent interventions and instrumentalize the work, in the future, of several professionals who work with these individuals, either in a clinical context or in a school context.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Nos últimos anos, pesquisas vêm destacando que déficits nas Funções Executivas (FE) se constituem uma das principais características do funcionamento cognitivo de indivíduos com Síndrome de Down (SD). Identificam-se dificuldades dos mesmos na iniciação de atividades, construção de planos e estratégias, atualização e monitoramento destas, manutenção do foco e persistência, flexibilidade cognitiva, dentre outros aspectos. Tais prejuízos impactam negativamente a vida destes indivíduos e apontam para necessidade da elaboração de programas interventivos que busquem estimular o desenvolvimento das FE neste subgrupo clínico, uma vez que estas funções são consideradas fundamentais para a realização de diversas atividades do nosso cotidiano, notadamente para a aprendizagem e sucesso escolar. Neste sentido, a presente pesquisa teve como objetivo adaptar para o grupo clínico da SD um programa interventivo das FE. A fim de atingir tal objetivo, foram realizados dois estudos, o estudo I: I) Investigação de aspectos do Fenótipo Neuropsicológico de crianças escolares com SD; e o estudo II) PIAFEX-SD: Programa de Intervenção em Autorregulação e Funções Executivas para a SD. Participaram do estudo I 31 crianças diagnosticadas com SD, com idades entre 7 e 12 anos, de ambos os sexos, bem como os seus respectivos pais ou responsáveis. Estas foram avaliadas através de escalas e medidas neuropsicológicas específicas. Os resultados deste estudo contribuíram para a compreensão de aspectos do fenótipo neuropsicológico da SD, aspectos estes que foram levados em consideração na adaptação da intervenção no estudo II, tais como: os déficits pronunciados na memória de trabalho, o nível de desenvolvimento rebaixado nos diferentes subdomínios do comportamento adaptativo, as idades mentais observadas e a presença ou ausência de sintomas do TDAH nestas crianças e o impacto disto para seus resultados. Em relação ao estudo II, inicialmente, foi realizada uma análise detalhada dos módulos e atividades que compõem o PIAFEX, objetivando selecioná-las e adaptá-las ao fenótipo neuropsicológico da SD. Finalizada a adaptação, o programa foi enviado para juízes especialistas na SD, com o objetivo de avaliá-lo, bem como possibilitar o seu aprimoramento e/ou reformulação. Após a síntese da avaliação dos juízes, o programa foi pilotado com um grupo de três crianças com SD, a fim de verificar sua adequabilidade para este grupo clínico. Ao final deste processo, o programa foi revisado e realizadas as últimas modificações. Esta pesquisa possibilitou a disponibilização para a população com SD de um programa de intervenção inédito voltado para a estimulação das FE.
