and tends to affect mainly the poorest and most vulnerable populations. In Brazil, the
disease had a predominantly rural distribution, but from the 1980s was its expansion
to urban and peri-urban areas of medium and large cities, like Natal-RN. The aim of
this study was to evaluate the factors associated with human infection with
Leishmania infantum and its spatial distribution in Natal-RN municipality. This is an
exploratory descriptive epidemiological study, ecological analysis, considering as the
unit of analysis neighborhoods (n = 36) that make up the municipality. A temporal
analysis was performed (1990-2014) for the demographic characteristics of cases of
human VL and VL-HIV coinfection, and spatial analysis (2007-2014) and correlation
with socioeconomic and environmental factors, canine infection and potentially
influencing entomological indicators transmission of Leishmania. The data of human
infection show strong endemism of VL in Natal. There was high density Lu.longipalpis
and high Leishmania infection in dogs over the years. Human VL, measured by the
number of case rate per 100,000 population, showed heterogeneous distribution and
spatial expansion over time, with the highest concentration of cases in the north of
the city. Males under the age of 4 years were the most affected and there was a
growing number of cases of VL-HIV coinfection. Socio-economic and structural
variables, which increase implies improving the standard of the neighborhood, such
as garbage collection, earning more than three minimum wages and literate
population, led to a reduction in the incidence rate of VL, while lack of sanitation and
income below the minimum wage, promoted its increase. VL in Natal has its
expansion associated with poverty and people in more vulnerable population, such
as children and immunocompromised, dwelling areas of recent development with
poor public infrastructure and the presence of infected dogs. |

A leishmaniose visceral (LV), ou calazar, é uma das doenças parasitárias mais letais e tende a afetar, principalmente, as populações mais pobres e mais vulneráveis. No Brasil, a doença tinha uma distribuição eminentemente rural, mas a partir da década de 1980 houve sua expansão para áreas urbanas e periurbanas de médio e grande porte, a exemplo de Natal-RN. O objetivo deste estudo foi analisar os fatores associados à infecção humana por Leishmania infantum e sua distribuição espacial no município de Natal-RN. Trata-se de um estudo epidemiológico, com análises de correlações ecológicas, considerando como unidade de análise os bairros (n=36) que compõem o município de Natal. Foi realizada uma análise temporal (1990 a 2014) das características demográficas dos casos de LV humana e de coinfecção LV-HIV, além de análises espaciais (2007 a 2014) e de correlação com fatores socioeconômicos e ambientais, infecção canina e indicadores entomológicos potencialmente influenciadores na transmissão da Leishmania. Os dados de infecção humana demonstram o forte endemismo da LV em Natal. Foram observadas elevada densidade de Lu. longipalpis e alta infecção por Leishmania em cães ao longo dos anos. A LV humana, medida pela taxa de número de casos por 100.000 habitantes, apresentou distribuição heterogênea e expansão espacial ao longo do tempo, com maior concentração dos casos na região norte da cidade. Pessoas do sexo masculino e com idade inferior a 4 anos foram as mais acometidas e observou-se um aumento crescente dos casos de coinfecção LV-HIV. As variáveis socioeconômicas e estruturais, cujo aumento implica em melhoria do padrão do bairro, como a coleta de lixo, renda maior que três salários mínimos e percentual de alfabetizados, acarretaram uma redução da taxa de incidência de LV, ao passo que ausência de saneamento e renda inferior a um salário mínimo, promoveram o seu aumento.
