The increase of the elderly population is a universal phenomenon. The
aging process demands a care network, among which are the nursing homes. The
institutionalization imposes changes in the routine of the elderly, especially in diet. A
priori dietary indexes can be used to evaluate the quality of the meal. The present
study aimed to evaluate the quality of the main meal (lunch) consumed by
institutionalized elderly people and compare two types of meal quality indexes. A
cross-sectional study carried out with 322-elderly people living in ten nursing homes
in Natal, Brazil, between 2013 and 2014. Bio-sociodemographic, nutritional status
and food consumption data were collected. Consumption was assessed by heavy
food registration for two nonconsecutive days. The nutritional quality of the lunch was
evaluated through the Meal Quality Index (IQR) and the Main Meal Quality Index
(MMQI). Descriptive analysis of the population was performed by absolute and
percentage frequencies. To assess the agreement between the two meal quality
indices, the Wilcoxon test was performed. The scores of the two indices were
categorized into quartiles and a concordance between them was tested by crossclassification
and weighted kappa. The proportion of categorized elderly people in the
concordant quartiles and opposing quartiles was calculated. The descriptive analysis
of the MMQI was performed by measures of central tendency, dispersion and
percentiles. All components of this index were calculated for the lunch meal. To
compare the medians of the MMQI as for-profit / non-profit nursing homes, use the
Mann-Whitney test for the independent samples. The study population was
predominantly female (73.98%), living in nonprofit ILPI (66.5%) and over 80 years
(58.5%). According to BMI, a large proportion of low weight was evidenced (42.9%).
The quality of the lunch obtained a median score of 53.51 points through the MMQI
and 54.67 points through the IQR, with no statistical difference between them (p =
0.723). Comparing the two indexes, the cross-classification by quartiles showed good
agreement, with an exact 48.8% and a disagreement of 3.4% (weighted Kappa =
0.477). The proportion of elderly in the same quartile or adjacent quartile was more
than 85%. According to the MMQI, low scores of consumption of vegetables, fruits,
fiber and energy density were observed; besides high scores on the components of
processed meats, desserts and sweetened drinks. The quality of the meal consumed
in the for-profit nursing homes was statistically better than in the non-profit (p
<0.0001). It is concluded that the two meal quality indices analyzed have a good
agreement between them. However, the MMQI presents a more detailed evaluation
because it has a greater number and combination of components, which allow to
evaluate both risk and protection factors for the development of chronic diseases.
The meal consumed by the institutionalized elderly needs improvements in the
consumption of fruits, vegetables, fiber and energy density. Thus, it is necessary to
develop public policies aimed at improving the nutrition of this group. |

O processo de envelhecimento demanda uma rede de cuidados, entre os quais se enquadram as Instituições de Longa Permanência para Idosos (ILPI). A institucionalização impõe alterações na rotina dos idosos, especialmente na alimentação. Índices dietéticos a priori podem ser utilizados para avaliar a qualidade da refeição. O presente estudo objetivou avaliar a qualidade da refeição principal (almoço) consumida por idosos institucionalizados e comparar dois tipos de índices de qualidade da refeição. Estudo transversal realizado com 322 idosos residentes em dez ILPI de Natal, RN, entre 2013 e 2014. Foram avaliados dados bio-sociodemográficos, de estado antropométrico e de consumo alimentar e dietético. O consumo foi avaliado por registro alimentar pesado, por dois dias não consecutivos. A qualidade nutricional do almoço foi avaliada por meio do Índice de Qualidade da Refeição – IQR e do Índice de Qualidade da Refeição Principal – Main Meal Quality Index (MMQI). A análise descritiva de variáveis de caracterização da população foi realizada por frequências absolutas e percentuais. Para avaliar a concordância entre os dois índices, realizouse o teste de Wilcoxon. Os escores de pontuação dos dois índices foram categorizados em quartis e a concordância entre eles foi testada por classificação cruzada e kappa ponderado. Avaliou-se também a proporção de idosos categorizados em quartis correspondentes e opostos. A análise descritiva do MMQI foi realizada por medidas de tendência central, de dispersão e percentis. Todos os componentes desse índice foram analisados separadamente para a refeição do almoço. Para comparar as medianas do MMQI entre as ILPI com/sem fins lucrativos, utilizou-se o teste de Mann-Whitney para amostras independentes. A população do estudo foi predominantemente feminina (73,98%), residente em ILPI sem fins lucrativos (66,5%) e com mais de 80 anos (58,5%). Conforme IMC foi evidenciado grande proporção de baixo peso (42,9%). A qualidade do almoço obteve escore mediano de 53,51 pontos por meio do MMQI e 54,67 pontos por meio do IQR, não havendo diferença estatística entre eles (p = 0,723). Em relação à comparação entre os dois índices estudados, a classificação cruzada por quartis mostrou boa concordância, sendo a exata de 48,8% e a discordância de 3,4% (Kappa ponderado = 0,447). A proporção de idosos no mesmo quartil ou no quartil adjacente foi de mais de 85%. De acordo com o MMQI, observaram-se baixas pontuações de consumo de verduras, frutas, fibra e densidade energética; além de altas pontuações (próximas dos escores máximos) nos componentes de carnes processadas, sobremesas e bebidas adoçadas. A qualidade da refeição consumida nas ILPI com fins lucrativos foi estatisticamente melhor do que nas sem fins lucrativos (p < 0,0001). Conclui-se que os dois índices de qualidade da refeição analisados possuem uma boa concordância entre si. Contudo, o MMQI apresenta uma avaliação mais detalhada por possuir um maior número e combinação de componentes, os quais permitem avaliar fatores tanto de risco quanto de proteção para o desenvolvimento de doenças crônicas não-transmissíveis. A refeição consumida pelos idosos institucionalizados precisa de melhorias no tocante ao consumo de frutas, verduras, fibra e densidade energética.
