description Chemotherapeutic drugs are medicines that require high-risk initiatives to prevent failures in care that cause unnecessary damage to patients. Perform a Multimodal Analysis of failures and effects (AMFE) to prospectively identify the risks related to the phase of the preparation and dispensing of chemotherapeutic drugs in a hospital. This is a descriptive study performed in an outpatient unit of a reference center in Oncology, philanthropist, from the city of Natal-RN. For the application of the tool is composed of a multidisciplinary team consisting of a mediator, four pharmacists, two nurses expert in the field and a nursing technician involved in the process. The steps for preparing and dispensing chemotherapeutic drugs were described graphically by means of a simple flowchart. Then were listed the possible failures of each subprocess using the Brainstorming method. Each failure was assessed using the array of risk score. According to the score obtained was used a decision tree to detect which flaws needed interventions. From the results obtained were proposed interventions and monitoring indicators. 17 failure modes were identified in steps of preparation and dispensation of chemotherapeutic drugs. Among the 17 failure modes, two have been identified as high risk. The failure mode "to change the output window of the Medicine" potential causes: lack of attention, lack of signs in the window and lack of knowledge of the process. The failure mode "wrong Calculation of the dose of intrathecal drug" had as potential causes: defective calculator, change in presentation of the medicinal product and work overload. The following interventions have been proposed: stipulate limits of meds to be handled at a time, the Windows flag internally and externally, containing work instructions and perform the check calculation of double medicine intrathecal and record in own printed. The AMFE tool proved to be a valid method to improve patient safety, as it allows a prospective analysis in the process of chemotherapy medication preparation and dispensing phase, with the objective of identifying potential failures and their associated causes, and formulate strategies for fixing such vulnerabilities.

Quimioterápicos são medicamentos de alto risco que exigem iniciativas para evitar falhas no cuidado que ocasionem danos desnecessários aos pacientes. Realizar uma Análise Multimodal de Falhas e Efeitos (AMFE) para identificar prospectivamente os riscos relacionados à fase do preparo e dispensação de medicamentos quimioterápicos em uma instituição hospitalar. Trata-se de um estudo descritivo realizado em uma unidade ambulatorial de um centro de referência em oncologia, filantrópico, da cidade de Natal – RN. Para a aplicação da ferramenta foi composta uma equipe multidisciplinar formada por um mediador, quatro farmacêuticos, dois enfermeiros especialista na área e um técnico de enfermagem envolvidos no processo. As etapas de preparo e dispensação de quimioterápicos foram descritas graficamente por meio de um fluxograma simples. Em seguida foram listadas as possíveis falhas de cada subprocesso utilizando o método Brainstorming. Cada falha foi avaliada utilizando-se a matriz de pontuação do risco. De acordo com a pontuação obtida foi utilizado uma árvore de decisão para detectar quais falhas necessitavam de intervenções. À partir dos resultados obtidos foram propostas intervenções e indicadores de monitoramento. Foram identificados 17 modos de falha nas etapas de preparo e dispensação de medicamentos quimioterápicos. Dentre os 17 modos de falha, dois foram identificados como alto risco. O modo de falha 1- Trocar a Janela de Saída do Medicamento, teve como causas potenciais: falta de atenção, falta de sinalização na janela e falta de conhecimento do processo. O modo de falha 2- Cálculo errado da dose de medicamento intratecal, teve como causas potenciais: calculadora com defeito, mudança na apresentação do medicamento e sobrecarga de trabalho. Foram propostas as seguintes intervenções: estipular limites de medicações a serem manipuladas por vez, sinalizar as janelas interna e externamente, contendo instruções de trabalho e realizar dupla checagem do cálculo do medicamento intratecal e registro em impresso próprio.
